A comprehensive, evidence-based clinical guideline for the risk assessment, prevention, diagnosis, and treatment of venous thromboembolism (VTE) in patients with cancer, including deep vein thrombosis (DVT), pulmonary embolism (PE), and central venous access device (CVAD)-related thrombosis.

Cancer-associated thrombosis (CAT) is the second leading cause of death in patients with cancer after the malignancy itself. Patients with cancer face a four- to seven-fold increased risk of VTE compared with the general population, and this risk is further amplified by surgery, hospitalization, systemic anticancer therapy, and the presence of central venous access devices. The clinical management of thrombosis in cancer patients requires specialized approaches that balance the high recurrence risk against elevated bleeding risk, account for drug interactions with anticancer agents, and address unique situations including thrombocytopenia, brain metastases, gastrointestinal malignancies, and catheter-related events.1

This guideline synthesizes recommendations from six major evidence-based sources published between 2021 and 2024, rewritten in original language for clinical reference. It covers the full spectrum of cancer-associated VTE management from pathophysiology and risk stratification through acute treatment, long-term anticoagulation, and management of special populations.2 3 4 5 6 7
